For insurance purposes, we require all guests at the studio to sign a waiver form.

LIABILITY WAIVER FORM

I acknowledge that I am using the facilities at the West Coast Photo Studio and that my activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By stating yes in the form, I assume all risks related to the use of any and all spaces used during the photo session at Studio D.

I agree to release and hold harmless Bob Garlick of DB Communications, Garlick Photography, and Studio D, including their agents and assigns, from any cause of action, claim, or demand now or in the future. I will not hold the people mentioned above or the facilities liable for any personal injury or personal property damage that may occur on the premises before, during, or after photo sessions.

Furthermore, I agree to obey Studio Drules and take full responsibility for my behaviour, in addition to any damage I may cause to the facilities and equipment utilized by Bob Garlick of DB Communications and Garlick Photography at Studio D.

I understand that Bob Garlick of DB Communications, Garlick Photography, and Studio D are licensed, accredited, and insured organizations.

If I should observe any unsafe conduct or conditions before, during, or after my use of the space, I agree to report the unsafe behaviour or conditions to Mr. Bob Garlick or a staff member at Studio D as soon as possible.

I agree to abide by this liability waiver and affirm that I have read and am in agreement with the contents of this waiver by choosing yes in this form.
